Please remember to bring the following:

  • ALL MRI films, reports, CT's, Bone scans or X-Rays that you might have or have had taken in the past.
  • Allow up to 3 hours once you have arrived at the Doctors office.
  • Consultations can range from $400 - $1050 depending on the amount of X-rays taken at our office.
  • If your insurance is an HMO or a POS please remember to bring your Referral and Doctor Directories in case we need to send you for some more testing or physical therapy. Knowing who your plan supports will help us to refer within your plan.
  • If you are on a HMO, POS, or PPO that requires a referral and you come to your appointment without your referral we may have to reschedule your appointment for a later date when you have had time to get the proper referral.
